§ 113.38 APPLICATION AND FEE.
   (A)   Application.
      (1) The application for a license shall contain the following information:
         (a) The name and address of the applicant, age, date, and place of birth;
         (b) Prior felony convictions of applicant, if any;
         (c) The place where the machine or device is to be displayed or operated and the business conducted at that place;
         (d) A description of the machine to be covered by the license, mechanical features, name of manufacturer, serial number.
      (2) No license shall be issued to any applicant unless he shall be over 18 years of age and a citizen of the United States.

   (B)   License fees.
      (1) Every applicant, before being granted a license, shall pay an annual license fee in the amount of $10 per machine for the privilege of operating or maintaining for operation each mechanical amusement device as defined in § 113.35.
      (2) Each license shall expire one year from the date of issuance.
