(A) The person, firm or corporation owning or controlling the business establishment shall supply a blank identification card to each parent or guardian who makes a request for same.
(B) The person, firm or corporation owning or controlling the business as hereinbefore defined shall further keep and maintain a registration book in which each minor shall sign indicating the date the minor entered the premises, the time of entrance and the time of departure therefrom.
