§ 50.38 REQUIREMENTS.
   The following requirements shall be met for all connections to the Town of Fremont wastewater treatment infrastructure by users from the Town of Clear Lake, Indiana.
   (A)   (1)   Tap fees of $1,000 for the Town of Clear Lake and $250 for the Town of Fremont, for a total of $1,250, shall be paid to the Town of Fremont.
      (2)   This fee is subject to change based on the calculated flow.
   (B)   (1)   All laterals shall be a 4-inch SDR-26 slip joint water grade pipe or greater meeting ASTM D2241 specifications.
      (2)   Installation shall be at no more than 1-1/4 inches of slope in 10 feet.
      (3)   Laterals shall not be placed closer than 10 feet from any private water well.
      (4)   A clean out is required and shall be placed no more than 3 feet outside of the foundation wall and shall extend above ground.
   (C)   (1)   The grinder pump and tank shall be an Environmental-1 Model GP2012.
      (2)   Placement of the tank shall be at the direction of the Wastewater Superintendent of the Town of Fremont or 1 of the Superintendent’s agents.
      (3)   The grinder tank shall be installed plumb in a bed of no less than 12 inches of pea stone and anchored with concrete in a manner set forth by the Environmental-1 Corporation specifications, which may be obtained at the office of the Fremont Wastewater Superintendent.
      (4)   Backfill shall be of pea stone or sand to a point 12 inches above the discharge line. All open cut lines shall be bedded in sand and a cover of no less than 6 inches of sand. All plumbing shall meet state and local codes.
   (D)   An audio and visual alarm box shall be placed so that it will be in the line of sight of a public roadway when possible. The alarm box shall in no event be hidden from view.
   (E)   (1)   A 10-3 with ground gauge wire shall be run from the circuit box to the alarm panel. This wire shall be on a separate circuit and be on a 30-amp breaker.
      (2)   An Environment-1 supplied power cord from the alarm box to the grinder pump shall be installed in 1 inch PVC conduit and buried. All exposed wiring from the circuit breaker panel to the alarm box shall be in conduit. All electrical work shall meet state and local codes.
   (F)   (1)   A 1-1/4-inch SDR11 discharge line is required from the grinder tank to the point of tap. This line shall be buried no less than 46 inches below final grade.
      (2)   The tap saddle shall be a power-seal stainless steel model 3131AS. The Wastewater Superintendent must approve any equivalent tap saddle before excavation begins. A curb stop valve shall be placed in-line on the property line on the homeowner's side.
      (3)   A swing check valve shall be placed next to the point of tap. A tracer wire is required on the discharge line from the point of tap to the grinder tank. Taps are the responsibility of the homeowner.
   (G)   Taps are allowed between 8:00 a.m. to 2:00 p.m. Monday through Thursday. The Wastewater Superintendent or an agent must be present when the tap is made. No taps are allowed 24 hours before any holiday recognized by the Town of Fremont without prior approval of the Wastewater Superintendent.
   (H)   Non-residential structures and multi-units shall be subject to guidelines set forth under the Town of Fremont Sewer Use Chapter and reviewed by the Wastewater Superintendent.
   (I)   All hookups are subject to the Town of Fremont Sewer Use Ordinance.
   (J)   All work shall be inspected and approved by the Town of Fremont Wastewater Superintendent or 1 of the department's agents. All work will be left uncovered until inspected and passed. In the event that work is covered and cannot be viewed for inspection, a $100 re-inspection fee will be charged to the landowner.
