Fund #7704, Dental Reimbursement and Payment Fund, is established for receiving and dispersing funds for a dental fund for employees and dependants covered under the employee health plan and not covered under another dental health plan and is to be spent without appropriation. An eligible dependent is for those that are 21 and younger who are not covered by the medical health plan and not covered by another insurance. This reimbursement can be for cleanings, x-rays or dental work. This is a reimbursement of one time per year and must include the original receipt, not a credit card receipt, with the patient’s name, date of service and payment amount. Reimbursement will not be made from a statement. Reimbursement will not be paid if covered by another dental insurance.
