§ 111.17 APPLICATION.
   (A)   All applicants for a license under this chapter shall file a written sworn application signed by the applicant or his or her agent.
   (B)   In addition to any information the Clerk-Treasurer may deem necessary for the effective implementation of this chapter, the application shall contain, and the applicant shall provide the following:
      (1)   The name and permanent address of the applicant;
      (2)   The local address or addresses of the applicant;
      (3)   If the applicant is a partnership, the names of the partners; and if the applicant is a corporation, the name of the president of the corporation and the state of incorporation;
      (4)   A statement as to whether the business will be conducted from fixed locations, and if so, the addresses of those locations;
      (5)   The length of time for which the applicant is requesting the license. (Any part of a day shall be considered a full day and licenses shall be issued only for consecutive days);
      (6)   Fingerprints of the persons authorized by the applicant to transact business under the license;
      (7)   A statement of the nature and character of the goods, wares or merchandise to be sold;
      (8)   A listing of communities the applicant has conducted transient business in within the last 6 months prior to the date of application;
      (9)   The state identification number of the business, if required; and
      (10)   A statement of whether the business holds a current Indiana retail merchant’s certificate, and if so, the name, mailing address and main location of the business the license is issued to, and the date of expiration if a license is required.
