Any SIU who violates the following conditions of this chapter, or applicable State and Federal regulation, is subject to having his Wastewater Contribution Permit revoked in accordance with the procedures of Section 922.04(a)(1).
(a) Failure of SIU to factually report the wastewater constituents and characteristics of his discharge;
(b) Failure of SIU to report significant changes in operations, or wastewater characteristics and constituents;
(c) Refusal of reasonable access to the SIU's premises for the purpose of inspection, sampling or monitoring;
(d) Violations of conditions of the permit.
