Any registrant under this chapter shall submit to the Police Department a sworn statement in writing on a form furnished by the Police Department giving his complete identification, his signature, his permanent home address and full local address, the nature of the products or services in which he is interested, the names of the manufacturers of such products or of the organization which he is representing, the proposed method of operation in the City, a recent photograph of himself and whether he has been convicted of any crime, including the nature and details thereof.
