351.99 PENALTY AND WAIVER.
   (a)   Whoever violates any provision of this chapter is guilty of a minor misdemeanor.
   (b)   Each owner or operator of a vehicle who receives a notice or notices of violation of this chapter, except a violation of 351.04(f), shall report to the Municipal Court within seventy- two hours of the time of the notice or notices and pay five dollars ($5.00) for each violation. The failure of the owner or operator of a vehicle to make such payment shall render him subject to an additional penalty of five dollars ($5.00) for each violation which was not paid within seventy-two hours of the time of the notice.

   (c)   Each owner or operator of a vehicle who receives a notice or notices of violation of Section 351.04(f) shall report to the Municipal Court within seventy-two hours of the time of the notice or notices and pay one hundred dollars ($100.00) for each Handicapped Zone violation.
   (d)   If the owner or operator of a vehicle fails to pay such fine and additional penalty, then the Municipal Court shall notify such owner or operator by mail and if such owner or operator fails to make payment of such fine and penalty within ten days of the date of mailing such notification by the Municipal Court, shall be subject to the penalty provided by subsection (a) hereof. In order that the offender shall have the right to pay a fine and penalty as provided herein, such offender shall sign the following waiver and plea:
“I, the undersigned, having been informed of my constitutional right to trial and to consult an attorney at law and the meaning of the plea of guilty and waiver of trial, do hereby PLEAD GUILTY to said offense as charged; waive my right to a trial by the Court, and agree to pay the penalty prescribed by the ordinance.”
   If the violation notice is returned to the Municipal Court unsigned accompanied with the full payment of the fine and penalty as provided herein, then such receipt of the violation notice and payment shall be equivalent to the receipt of a signed waiver and plea.
