Every application for a license shall be made to the city administrator on forms supplied by the city. The application shall state the applicant's name, address, and telephone number, the name of the business, if different, the address and telephone number of the premises on or from which the business is to be conducted, any other business operated on or from the same premises, the type of license applied for, and such other information as may be required by this code or by the city council. All applications shall be accompanied by payment in full of the license fee. No person shall knowingly make a false statement in any license application. (2008 Code)
