Within ten days of the issuance, denial, suspension or revocation of a license, the village shall notify the Illinois Department of Public Health of the issuance or denial. If a license is issued, the notice shall include the facility name, address, the date of issuance and the number of beds by level of care. If a license is denied, suspended or revoked, the notice shall indicate the reason therefor and the current status of the license application.
