Application for a license shall be made to and filed with the Village President, shall be in writing, under oath and signed by the applicant, if an individual, by at least two partners if a partnership or the president or manager and the secretary if a corporation or limited liability company. Such application shall be presented not less than ten days prior to the date issuance is desired and shall be substantially in one of the following forms hereby adopted and referred to in this chapter by reference:
(A) Application for new retailer's liquor license.
(B) Application for renewed retailer's liquor license.
