(A) Applications for all licenses shall be made to the village in writing on the form provided for that purpose. The form is attached and may, from time to time, be amended by the village. Each application shall contain:
(1) Business name;
(2) A description of the type of business;
(3) The location or proposed location of the business;
(4) Business telephone number;
(5) Business owner; and
(6) Evidence that the required State Retailer’s Occupation Tax, Service Occupation Tax, or Use Tax as appropriate, has been paid for the preceding year.
(B) In addition, each application shall contain:
(1) If the applicant is an individual (sole proprietorship), the application shall contain the applicant’s name, residence address, and residential telephone number.
(2) If the applicant is a partnership or other non-corporate business entity, the application shall contain the name and residential address, and residential telephone number of each partner, principal, or member thereof.
(3) If the applicant is a corporation, the application shall contain the name, residential address, and residential telephone number of each principal officer and the registered agent thereof.
(4) If the applicant is an unincorporated association, the application shall contain the name, residential address, and residential telephone number of the spokesman.
(C) The Code Enforcement Officer shall make the determination what type of license is required for each respective applicant. This determination shall be made after review of the information provided in each application. The determination of the Code Enforcement Officer may be appealed to the Community Development Director, and then to the Village Administrator. If no resolution is reached, the determination may be appealed to the Village President whose decision shall be final.
(D) All establishments desiring a business license shall file an application with the village not less than 30 days prior to the time said establishments desire to commence operation. Thereafter, prior to continued operation of the establishment in subsequent years, a renewal application shall be filed not less than 30 days prior to expiration of the license. In the event such renewal application is not filed by this date, a late charge to be set by the Board of Trustees shall be charged. Delinquency can result in suspension or revocation of a business license.
(E) It shall be the duty of the Code Enforcement Officer to cause a written notice to be mailed by regular or electronic mail to each licensee advising the licensee when a license expires, advising that a renewal is required, and shall specify the amount of the license fee and the penalty for failure to procure a license in accordance with the provisions of this chapter.
(F) All Class A license holders shall have a monthly pest control program performed by a certified state of Illinois pest control exterminator. Class A business license holders shall show proof that they have a pest control contract, for a period no less than 12 months, with a certified state of Illinois pest control exterminator.
