Said residents and/or owners shall register the off-road vehicle with the village on a form provided by the Village Clerk which shall contain (1) the name of the registrant, (2) the registrant's address, (3) the registrant's social security number, (4) the registrant's telephone number, (5) a description of the vehicle being registered, and (6) the primary operators of said vehicle.
