3-9-6: REVOCATION OF LICENSE:
   A.   Agency's Grounds: The license of a servicing agency shall be revoked, if such agency:
      1.   Permits gambling of any kind to occur upon its business premises, including bets or wagers on horse races.
      2.   Refuses admission to inspection by a duly authorized representative of the Village or Chief of Police.
      3.   Having failed to maintain the records required by this Chapter, fails, within a period of seven (7) days to cure the default, unless such licensee, its agents, servants, and employees be wholly without fault both as to the original default and as to the failure to rectify it.
      4.   Fails to record a transaction upon an official transaction permit, employs a document to record such transaction upon which no transaction permit fee has been paid, or records a transaction upon a forced, counterfeit or used transaction permit.
      5.   Employs transaction permits which are not serially numbered or employs transaction permits upon which serial numbers are repeated.
      6.   Makes any false report or record required to be made hereunder, knowing the same to be false when made.
      7.   Failed to maintain a true record of unclaimed sums due its customers, or failed to maintain an adequate fund from which such unclaimed funds could thereafter be redeemed by customers.
      8.   Permitted the sale or consumption of liquor upon its business premises.
   B.   Hearing: Such revocation shall take effect only after the licensee shall be given written charges against him and a hearing held on said charges by the village president. If the president finds after hearing all of the evidence presented that the licensee has violated the provisions of this code or any state law, he may then revoke said license. Said revocation shall be accompanied by a written statement by the president setting forth his reasons for revoking the license. (Ord. 81-12, 4-20-1981)
   C.   Disposition Of Unused Transaction Permits: Upon revocation of any servicing agency license, or upon abandonment or nonrenewal of such license, a licensee hereunder will surrender to the village treasurer all unused permits, receiving in return therefor, the full amount paid to the village for such permits less ten percent (10%) of such transaction permit fees, which will constitute a handling cost owing to the village; provided that nothing herein contained shall deprive a licensee of the right of surrender of such unused permits at any time and receipt of all fees paid therefor, less the ten percent (10%) handling cost.
