Every person desiring to engage in soliciting from persons in residences of the Village shall make written application for a certificate of registration as provided herein. The application for the license required in Section 3-4-3 preceding shall constitute the application for a certificate of registration. For the purpose of providing a certificate of registration required by this Section, said application shall also require the applicant to provide the following information in addition to the information required in the application for license as required in Section 3-4-3C:
A. Address of place of residence during the past three (3) years if other than present address.
B. Age of applicant and marital status, and if married, the name of spouse.
C. Physical description of the applicant.
D. Name and address of the person, firm, corporation or association by whom the applicant is employed, or represents, and the length of time of such employment or representation.
E. Name and address of employer during the past three (3) years if other than the present employer.
F. Period of time for which the certificate is applied for.
G. The date, or approximate date, of the latest previous application for certificate under this Chapter, if any.
H. Whether a certificate of registration issued to the applicant under this Chapter has ever been revoked.
I. Whether the applicant has ever been convicted of a violation of any of the provisions of this Chapter, or the ordinance of any other Illinois municipality regulating soliciting.
J. Whether the applicant has ever been convicted of a commission of a felony under the laws of the State of Illinois or any other State or Federal law of the United States.
K. Such additional information as the Chief of Police may deem necessary to process the application. (Ord. 89-52, 6-19-89)
