3-1-2: APPLICATION AND NONREFUNDABLE DEPOSIT:
   A.   Licenses; Permits; Fee: Applications for all licenses and permits required by ordinance shall be made in writing to the village clerk in the absence of provision to the contrary. Each application shall contain: 1) the name of the applicant, 2) the permit or license desired, 3) the location to be used, if any, and 4) the time covered. Each application also shall contain the number of the certificate of registration required under the retailers' occupation tax act, service occupational tax act, and/or use tax act, if applicable 1 . Each application shall contain such additional information as may be needed for the proper guidance of the village officials in the issuing of the license or permit applied for. The application shall be accompanied by a nonrefundable deposit of fifty dollars ($50.00). The balance of the license fee, if any, shall be payable by the applicant to the village upon issuance of the requisite license.
   B.   Forms And Signature: Forms for all licenses and permits, and applications therefor, shall be prepared and kept on file by the village clerk. Each license or permit issued shall bear the signature of the village president in the absence of any provisions to the contrary. (Ord. 94-13, 6-6-1994)
   C.   Exposition Centers: For exposition centers, where more than one seller of merchandise will operate from the same premises, the holder of the business license for the premises shall supply the number of his certificate of registration required under the retailers' occupation tax act, service occupational tax act and/or use tax act, if applicable, and shall on a weekly basis supply to the village clerk the name, address, certificate of registration number (as described above) and type of products to be sold, for all sellers of merchandise who will be actively selling merchandise during the following week. For sellers of merchandise who claim to not be required to have a certificate of registration number (as described above), the holder of the business license for the premises shall supply all data required above except the certificate of registration number, along with an explanation of why no such certificate of registration number is required, and the holder of the business license for the premises shall be responsible for the accuracy of the information supplied.    D.   Video Gaming Facilities: Applications for licenses for video gaming facilities shall be evaluated and regulated by 230 Illinois Compiled Statutes 40/1 et seq. (video gaming act), as said act may be amended from time to time, and said act and the regulations promulgated thereunder are adopted as regulations under this code. (Ord. 2012-14, 6-12-2012)
