Applications for licenses for public vehicles shall be made in writing to the City Manager, shall set forth the name and address of the applicant, the trade name under which he does business, the identification of the vehicle for which a license is desired, and, in the event the applicant is a taxicab company, shall list the number and identification of each vehicle to be operated in the city in pursuance of the license to be issued to such applicant, and any other information required by the City Manager.
