(A) Applications shall be made on forms provided by the Commission. These forms shall require the applicant, under oath or affirmation, to state name, address, and other information as may reasonably be required concerning education, experience, health, physical and mental capacity, and felony conviction record. (B) All applications shall be completed, signed, and sworn or affirmed to, and filed in the Human Resources office not later than the date specified in the announcement of the examination.
