(A) To obtain a license to allow teen dances at locations containing coin-operated amusement devices subject to §§ 112.10 through 112.12, the person, firm, corporation, or other entity operating the premises on which the activity is proposed shall submit an application to the City Coordinator on forms provided by that office which shall include, at a minimum, the following information:
(1) An activity plan clearly stating in detail the nature and type of all activities to be conducted on the premises during operation of the teen dance.
(2) Proposed hours of all of these activities with an acknowledgement of understanding that any license that might be issued would be limited to those proposed hours set forth in the application.
(3) An applicant must identify and explain in detail efforts that have been made to coordinate the teen dance activities with local schools or recognized sponsors of schools.
(4) A statement as to the maximum capacity for the facility or premises which is proposed for use of the activities, and information concerning the size of the facilities as well as seating arrangements for the facility during the proposed hours. Any applicant shall allow duly authorized representatives of the Volunteer Fire Department, Police Department, and Building Inspector's Office to determine whether the proposed facility meets requirements of any other codes for this kind of activity.
(5) The method and plan for management control and supervision must be included within the application and the number of staff, employees, and chaperons to be present at the facility shall be provided. Management control and supervision must take into account all areas of the facility open to the public, including parking areas.
(6) All applicants shall provide the names, addresses, and social security numbers of those persons in charge of the operation of the teen dance, as well as all officers and directors of any corporation that might be involved in the operation of either the teen dance or the facility proposed for use.
(B) Upon receiving the application, it shall be forwarded by the City Coordinator's office to the Volunteer Fire Department's offices for review and approval. It shall also be forwarded to the Police Chief's office, who shall review and investigate those matters set forth in the application to determine whether the proposed method of management control and supervision are adequate.
