The person applying for such license shall file, with the Clerk, a statement of the number of concessions to be operated and the number of days the circus, menagerie, or carnival will be operated and shall accompany this statement with a certified check, cash, or money order for a sum equal to $1 per day for each concession multiplied by the number of days the concessions are to be operated, and, if the statement includes a tent show, the application shall be accompanied by a certified check, cash, or money order for a sum equivalent to $5 per performance.
