§ 50.04 OBTAINING SERVICE.
   (A)   Persons wishing to obtain city supplied utilities and services will make written application for said utilities and services on forms provided by the city office.

   (B)   It shall be unlawful for any person to obtain utilities or services from the city under an assumed name or any name other than his or her true name.
