A permit fee of $250 shall be paid to the Treasurer at the time the application is filed, which fee shall entitle the applicant to a three-fourths inch meter, connection to main, curb stop, and box, gate valve, and 60 feet of three-fourths inch plastic service pipe, with additional fees to be paid by the applicant for additional materials provided by the village.
