Contact the village office for a permit application form (an application for permit must be submitted 60 days prior to setting up). The applicant shall provide:
(A) Name and address (as well as years at that address);
(B) Phone and cell phone numbers;
(C) E-mail address;
(D) Information as to age, gender, weight, eye and hair color, and date of birth;
(E) Social Security number;
(F) Driver’s license number and state licensure, vehicle registration, and state of registration;
(G) Provide a through description of the business and how it would operate while in the village;
(H) If employed, the owner’s full name (business/company), address, and phone number, as well as a copy of all licenses, registrations, and health certificate/licenses, as well as pertinent tax information needed to operate a mobile food truck/trailer. Credentials establishing that the applicant has a relationship with said business/company, including authority by the employer authorizing the applicant to represent said employer, is required;
(I) Starting and ending dates the applicant (business/company) plans to be in the village;
(J) A listing of others who will be working with him or her in the village; and
(K) The applicant must indicate if the applicant, or anyone working with him or her in the village, have ever been convicted of a felony under state law or laws of the United States, or is a registered sex offender.
