Contact the village office for a permit application form (an application for permit must be submitted 60 days before). The applicant shall provided:
(A) His or her name and address (as well as years at that address);
(B) Phone and cell phone numbers;
(C) E-mail address;
(D) Information as to age, gender, weight, eye and hair color, and date of birth;
(E) Social Security number;
(F) Driver’s license number and state of licensure, vehicle registration, and state of registration;
(G) Provide a thorough description of goods, merchandise, or service to be peddled or solicited, together with copies of all brochures, sales materials, applications, order forms, contracts, disclosures, business cards, and any other documents to be used by peddler/solicitor while in the village;
(H) If employed, the owner’s name (business/company) and address, as well as any licenses or any pertinent tax information (tax ID number, insurance, and the like) is needed, as well as credentials establishing such relationship with said business/company, including the authority by the employer authorizing the applicant to represent said employer;
(I) Starting and ending dates the applicant (business/company) plans to be in the village;
(J) A listing of others who will be working with said applicant (names, addresses, and description of vehicles); and
(K) The applicant must indicate if he or she or anyone working with him or her in the village, have ever been convicted of a felony under state’s law or the laws of the United States
