The application for a license under this section shall be made in duplicate to the Clerk or his or her designated representative, upon forms to be supplied by her for that purpose. The application shall set forth the names and addresses of the applicant or of a corporation, partnership or association, the principal officers and registered agent thereof and their addresses, and the owners of the machines. The application shall state the type of license applied for, the place where the devices are to be displayed or operated, the business conducted at that place, a description of the device or devices to be covered by the license, giving the mechanical features, the name of the manufacturer and serial number, the employment or business of the applicant during the previous five years, whether the applicant was ever convicted of a crime and the records thereof, and the true name of each owner and part owner and each person who proposes to share in the profits and income from operation of the coin-operated amusement devices for which the license is sought.
