The application for a license shall be made to the city clerk on a form provided by the city clerk. The application shall set forth the name of the applicant, his residence, place of business, the nature of the business, and the amount of the license fee prescribed by this chapter, and shall be accompanied by a deposit of the license fee by the applicant. The applicant or his agent shall personally sign every application. Every application shall be accompanied by a statement from the fire department stating an inspection of the premises of said business has been completed and all regulations complied to.
