In the event that the Animal Control Officer receives a complaint concerning a cat or a dog, which has bitten a human being or attacked and killed another animal, a sworn complaint must be submitted with the following information:
(A) The name, address and telephone number of the complainant and any other witnesses to the incident;
(B) The date, time and location of the incident;
(C) The description of the cat or dog;
(D) The name, address and telephone number of the cat or dog owner;
(E) A statement that the cat or dog attacked and/or killed another animal;
(F) A statement that the cat or dog attacked and/or bit another human being;
(G) A statement that the cat or dog has exhibited vicious propensities in past conduct; and
(H) Any other facts and circumstances relevant to the reported incident.
