A. An application for a business license to conduct a taxicab service shall be filed in writing with Farr West City. The license application shall include:
1. The name and location of the applicant's principal place of business and any other office located in the state of Utah.
2. The number of vehicles to be used in the taxicab service, a description of each vehicle, its license plate number and vehicle identification number (VIN).
3. Provide a current (within 6 months) bureau of criminal identification (BCI) background check for applicant.
4. Provide competent evidence of valid liability insurance coverage for each driver and each taxicab service vehicle as required by state code and maintain continuous vehicle insurance coverage while the vehicle is operational at the minimum levels of coverage required by state statute. 5. Evidence that all drivers have a current motor vehicle chauffeur's permit issued by the state of Utah, authorizing the transportation of passengers (a class D license with a Z endorsement).
6. If any of the foregoing information changes, the applicant or licensee, as the case may be, shall deliver current information to the Farr West City office within ten (10) business days following the change.
7. Any taxicab licensed through another jurisdiction within the state of Utah shall be recognized as properly licensed in the city of Farr West. (Ord. 15-08)
