§ 110.04 APPLICATION FOR REGISTRATION LICENSE.
   (A)   Each itinerant merchant or peddler shall, prior to engaging in sales or related activities, apply to the Clerk-Treasurer for a license to do so.
   (B)   Each applicant shall furnish to the Clerk-Treasurer the following information:
      (1)   Name;
      (2)   Permanent address;
      (3)   Local address;
      (4)   Date and place of birth;
      (5)   Merchandise or produce to be sold:
      (6)   Name and address of employer;
      (7)   Name and address of immediate supervisor;
      (8)   Copy of current driver’s license or photo identification; and
      (9)   Make, model, year, color and license plate number of all vehicles being used.
