§ 39.37 FORM OF THE STATEMENT OF FINANCIAL INTERESTS.
   The statement of financial interests shall be filed on a form prescribed by the Board of Ethics, or the administrative official designated by the Board of Ethics. Such prescribed forms shall be made available at the office of the City Clerk.
