§ 111.21 APPLICATION.
   The application for a permit required by the provisions of this subchapter shall include the following:
   (A)   A statement as to whether or not the applicant has been convicted of any crime, misdemeanor, or violation of any state or federal law or municipal ordinance or code other than traffic offenses; the nature of the offense; the punishment or penalty assessed therefor, if previously convicted; and the place of conviction;
   (B)   Whether the applicant, upon any sale or order, shall demand, accept, or receive payment, or deposit of money in advance of final delivery;
   (C)   The period of time the applicant wishes to engage in business within the city;
   (D)   The local and permanent addresses of the applicant;
   (E)   The local and permanent addresses and the name of the entity, if any, that the applicant represents;
   (F)   The kind of goods, wares, merchandise, food, beverage, or services the applicant wishes to peddle within the city;
   (G)   The last five cities or towns wherein the applicant has worked before coming to this city;
   (H)   The applicant’s state sales tax license number;
   (I)   The applicant’s date of birth and Social Security account number or other identifying number; and
   (J)   Contain a statement that the peddler or vendor covenants and agrees that it will protect and save and keep the city forever harmless from and indemnify against any penalty or damage or charges imposed for any violation of any laws or ordinances; occasioned by the neglect of peddler or vendor or those holding under peddler or vendor, and that peddler or vendor will at all times protect, indemnify, and save and keep harmless the city against and from any and all loss, cost, damage, or expense, arising out of or from the fault or negligence of peddler or vendor, causing injury to any person or damage to property and will protect, indemnify, and save and keep harmless the city against and from any and all claims, loss, cost, damage, or expense arising out of any failure of peddler or vendor in any respect to comply with and perform all the requirements and provisions hereof.
