A licensee shall report to the Town Council all leases to which it is a party on any premises where gaming is conducted not later than 30 days after the effective date of the lease and shall include the following information:
(A) The name, address and a brief statement of the nature of the business of the lessor;
(B) A brief description of the material terms of the lease; and
(C) A brief description of any business relationships between the licensee and the lessor other than by the license.
