510.02 LICENSE APPLICATION; FEE; REQUIREMENTS.
   (a)    No person, partnership, corporation, or other entity shall operate or conduct a billiard room, game room, poolroom or arcade for use by the general public within the Village for money or other reward without first obtaining an annual license therefor from the Chief of Police of the Village.
   (b)    Every person, partnership, corporation, or other entity shall make an application for said annual license, in writing, to the Chief, that shall be accompanied by an annual application fee as shall be determined by the Council of the Village, and shall set forth the following information:
      (1)    The name under which the business is to be conducted;
      (2)    The location the business is to be carried on;
      (3)    The name and address of any manager, adult supervisor, or other employee;
      (4)    The name and address of the owner or owners of the coin-operated amusement devices; and
      (5)    The name, address, and principal occupation of every person with an interest in the business. If the business is conducted by:
         A.   A sole proprietorship, the name, address, and principal occupation of that individual.
         B.   A firm or partnership, the names, addresses, and principal occupations of each member of the firm or partnership.
         C.   A corporation or Limited Liability Company, the names, addresses, and principal occupations of all officers, Directors, Members and/or Shareholders.
   (c)    The person, partnership, corporation, or other entity operating or conducting the business shall have a continuing duty to inform the Chief of Police as to changes in the information required by this section.
   (d)    If an application is not approved, the Chief of Police shall notify the applicant in writing, with reasons for rejection.
   (e)    The Chief of Police shall not issue such license to any person, partnership corporation, or other entity if any of the persons with an interest in the business or if any of the employees of the business have been convicted of a violation of a federal or state statute or of any local ordinance pertaining to gambling, theft, drug use or possession, or a crime of moral turpitude within five years preceding the subject application. In addition, as a prerequisite to the issuance of such annual license, an inspection shall be made of the premises by the Fire Division of the Village, the Chief Zoning Inspector of the Village and the Lake County Department of Public Health, and such approval must first be obtained from each of said agencies.
