(a) In addition to the application requirements contained in Chapter 810, every applicant must provide:
(1) A full and complete description of the inventory of the mechanical equipment to be utilized in the City for snow plowing including vehicle license numbers.
(2) A copy of the applicant's Auto Liability Insurance Certificate providing minimum coverage for combined bodily injury and property damage of five hundred thousand dollars ($500,000) per occurrence.
(3) Evidence of the applicant's registration with the Tax Administrator of the City to ensure payment of all City taxes accruing from the snow plowing operation.
(4) Such other information as required by the Service Director.
