848.03   LICENSE APPLICATIONS.
   (a)   An application for the license required by Section 848.01 shall be made to the Chief of Police, which application shall set forth the following:
      (1)   The name under which the business is to be conducted;
      (2)   The location at which the business is to be conducted;
      (3)   The name and address of any manager or other employee; and
      (4)   The name, address and principal occupation of every person with an interest in the business. If the business is conducted by a sole proprietorship, the application shall include the name, address and principal occupation of that individual. If the business is conducted by a firm or partnership, the application shall include the name, address and principal occupation of each member of the firm or partnership. If the business is conducted by a corporation, the application shall include the names, addresses and principal occupations of all officers and shareholders.
   (b)   An application to sell any item, effect, paraphernalia, accessory or thing which is designed or marketed for use with illegal cannabis or drugs shall be accompanied by affidavits from every person with an interest in the business, and each and every employee authorized to sell such items, that he or she has never been convict of a drug-related offense.
   (c)   The person operating or conducting the business shall inform the Chief of Police as to changes in persons having an interest in the business and employees of the business. This provision shall include the submission of affidavits for each such person or employee.
   (d)   The Chief shall not issue a license to an applicant if a person with an interest in the business, or an employee of the business, has ever been convicted of a drug-related offense.
