Upon the issuance of the license referred to in Section 830.02, the user of pesticides shall notify the Utilities Commissioner of the name, address and telephone number of the person, persons or business entity for whom or which he or she will be applying the pesticides, and of the generic chemical names of pesticides to be used, in addition to any other information the Utilities Commissioner may deem to be necessary.
