(A) Before any permit is issued by the city, an application shall be made on the forms as provided by the city and shall contain the following information:
(1) Name and address of applicant;
(2) Name of liability insurance carrier;
(3) Serial number, make, model and description of the golf cart or utility terrain vehicle;
(4) Proof of insurance coverage specifically for the vehicle to be operated pursuant to the permit;
(5) Photocopy of the applicant's current driver's license.
(B) Such other and further information as the city may require.
