3-8-5: INSURANCE:
   A.   Insurance Requirements: Every operator shall maintain in full force and effect, at all times, the following insurance policies:
      1.   General Liability:
         a.   Bodily injury liability: Limits shall be carried in an amount not less than one million dollars ($1,000,000.00) for any claim or to any claimant who has more than one claim for loss arising out of a single accident or occurrence; and not less than one million dollars ($1,000,000.00) to any claimant for his claim for any other loss arising out of a single accident or occurrence; and not less than two million dollars ($2,000,000.00) for any number of claims arising out of a single accident or occurrence.
         b.   Property damage liability: Limits shall be carried in an amount not less than three hundred thousand dollars ($300,000.00) for any claim or to any claimant who has more than one claim for loss arising out of a single accident or occurrence; and not less than three hundred thousand dollars ($300,000.00) to any claimant for his claim for any other loss arising out of a single accident or occurrence; and not less than two million dollars ($2,000,000.00) for any number of claims arising out of a single accident or occurrence.
      2.   Professional Liability:
         a.   Bodily injury liability: Limits shall be carried in an amount not less than one million dollars ($1,000,000.00) for any claim or to any claimant who has more than one claim for loss arising out of a single accident or occurrence; and not less than one million dollars ($1,000,000.00) to any claimant for his claim for any other loss arising out of a single accident or occurrence; and not less than two million dollars ($2,000,000.00) for any number of claims arising out of a single accident or occurrence.
         b.   Property damage liability: Limits shall be carried in an amount not less than three hundred thousand dollars ($300,000.00) for any claim or to any claimant who has more than one claim for loss arising out of a single accident or occurrence; and not less than three hundred thousand dollars ($300,000.00) to any claimant for his claim for any other loss arising out of a single accident or occurrence; and not less than one million dollars ($1,000,000.00) for any number of claims arising out of a single accident or occurrence.
      3.   Automobile Liability:
         a.   Liability: Limits shall be carried in an amount of not less than one million dollars ($1,000,000.00) to any claimant who has more than one claim for loss arising out of a single accident or occurrence; and not less than one million dollars ($1,000,000.00) to any claimant for his claim for any other loss arising out of a single accident or occurrence; and not less than one million dollars ($1,000,000.00) for any number of claims arising out of a single accident or occurrence.
         b.   Comprehensive and collision loss: Limits shall be carried in an amount of not less than actual cash value with deductibles of not more than one thousand dollars ($1,000.00) per vehicle.
         c.   Medical payments: Limits shall be carried in an amount of not less than one hundred thousand dollars ($100,000.00) to any claimant who has more than one claim for loss arising out of a single accident or occurrence; and not less than three hundred thousand dollars ($300,000.00) to any claimant for his claim for any other loss arising out of a single accident or occurrence.
      4.   Workers' Compensation: Limits shall be carried in an amount not less than the amount prescribed by the laws of the state of Oklahoma.
      5.   Contractual Liability: If the ambulance service operates a subscription program, limits shall be carried in an amount not less than the actual amount collected from the sale of such subscriptions.
      6.   Umbrella: An additional one million dollars ($1,000,000.00) umbrella policy must be provided.
   B.   Carriers: The insurance policies shall be executed by an insurance or indemnity carrier authorized to do business in the state of Oklahoma.
   C.   Notice Of Cancellation: All such certificates of insurance shall provide for a thirty (30) day cancellation notice to the city clerk.
   D.   Proof Of Insurance: The operator shall submit to the city clerk annually, or upon any change in insurance, certificates of insurance. (Ord. 2015-06, 2-3-2015)
