12-5-4: MAP CONTENTS:
   A.   Description:
      1.   Name of the subdivision.
      2.   Name of the subdivider, owner, engineer or surveyor.
      3.   Location by section, range, township, city, county, state.
   B.   Existing Conditions:
      1.   All plat boundaries.
      2.   Bearings and distances to the nearest established street lines, section corners or other recognized monuments which shall be accurately described on the plat.
      3.   Municipal, township, city, county or section lines accurately tied to the lines of the subdivision by distance and bearings.
      4.   Accurate location of all monuments.
   C.   Survey Data:
      1.   Length of all arcs, radii, internal angles, points of curvature and tangent bearings.
      2.   When lots are located on a curve or when side lot lines are at angles other than ninety degrees (90°), width at the building lines shall be shown.
   D.   Drafting Of Plat: Date of preparation, scale of map, north arrow.
   E.   Proposals:
      1.   All easements for rights of way provided for public services and/or utilities, and any limitations of such easements.
      2.   All lot numbers and lines, with accurate dimensions in feet and hundredths of feet and with bearings or angles to street and alley lines.
      3.   Accurate outlines of any areas to be dedicated or reserved for public use with the purpose indicated thereon.
      4.   Building setback lines, with dimensions.
   F.   Title Block Information:
      1.   Protective covenants shall be provided on separate recordable instruments.
      2.   Director of engineering services certificate as required.
      3.   Certification shall be furnished from the city clerk or county treasurer that all taxes and assessments have been paid on the land within the proposed subdivision.
      4.   Certification from the planning commission, and the mayor and board of commissioners.
      5.   Certification by a registered civil engineer or licensed land surveyor to the effect that the plat represents a survey made by him and that all monuments shown thereon actually exist, and that their location is correctly shown.
      6.   An acknowledgment by the owner or owners of his or their adoption of the plat, and of the dedication of streets and other public areas.
      7.   All proposed lots shown on the plat shall be sequentially numbered, according to the address system adopted by the city. Street addresses shall be assigned by the code administration department and shall be designated and printed on each proposed lot. Each plat shall then include a caveat and a disclaimer to read as follows:
Addresses shown on this plat are accurate as of the time the plat was filed of record. Addresses are subject to change and should never be relied upon in place of the legal description.
