252.06 GROUP HEALTH CARE PLAN.
   (a)   As an additional benefit for full-time regular and probationary employees, the City will provide a group health care plan(s) to include medical, hospitalization and major medical, prescription drug, and dental benefits. Payment for the premiums of such health care plan(s) shall be shared by the City and participating employees at a percentage deemed reasonable and appropriate by a Benefits Committee consisting of the Director of Personnel, the Director of Finance, and the City Manager.
   (b)   The group health care plan(s) may consist of a fully insured benefit program, a self-insured benefit program, or any combination thereof. The City Manager is hereby authorized to utilize the services of a broker, agent, or consultant in the collection of proposals and/or administration of said plan(s). Level of benefits; employee premium contributions, co-pays, deductibles, and out-of-pocket maximums and features of the plan(s) shall be reviewed on an annual basis and maybe adjusted in accordance with the cost of such plan(s) and the financial resources of the City.
   (c)   All full-time regular and probationary employees will be eligible to participate in the group health care plan(s) from the time of their appointment until such time as they are separated from employment, or are no longer in a full-time employment status. Part-time employees shall not be eligible to participate in the group health care plan(s). The premiums for the provided benefits, except for that portion of employee contributions, shall be paid from the same funds of the City which provide the wages for each participating employee.
