§ 150.15 DRIVEWAY PERMITS.
   (A)   Any person, firm or corporation wishing to construct or alter any driveway or entrance upon any public street or alley of the city shall first obtain a permit to do so by making application to the Clerk for a permit to do so at least seven days prior to the date on which the applicant wishes to begin construction thereof. Said application shall include the following:
      (1)   Application fee in the amount of $25.
      (2)   Statement as to the size, width and purpose for the proposed driveway or entrance.
      (3)   Drawing showing the exact location of the proposed driveway or entrance.
      (4)   Evidence of conformity with county and state requirements.
   (B)   The Highway Commissioner shall within five (5) days of proper application for a driveway permit being made direct the Clerk to issue the building permit and the Clerk shall issue the driveway or entrance permit applied for if the Highway Commissioner, upon inspection, is satisfied that the proposed driveway is in conformity with this and all other ordinances of the city. If the Highway Commissioner is not so satisfied then he/she shall give the applicant a written statement of why the permit is denied and the applicant may within 30 days thereafter re-apply without further application fee.
   (C)   No permit for a driveway or entrance shall be granted if it appears to the Highway Commissioner that the construction thereof would:
      (1)   Damage the public street or alleys.
      (2)   Interfere with the proper drainage or surface waters along said streets or alleys.
      (3)   Create a safety hazard to motorists or pedestrians using the public streets or alleys of the city.
   (D)   The standard for construction or alteration of such driveways or entrance ways shall be as follows: all culvert (drainage pipe) shall be a minimum of 12 inches in diameter, not less than 16 gauge in thickness, and made of galvanized corrugated steel. The minimum width for a simple dwelling entrance way or driveway shall be eighteen feet. All other entrance ways shall be a minimum of 24 feet in width. Stone or concrete headwalls shall be installed on each end of all culvert-drainage pipes.
