A. Form: All applications for licenses under this title, unless otherwise specified herein, shall be made to the City Clerk, in writing, upon blanks provided therefor.
B. Required Information: Each applicant shall make signed statements of the nature of the business; on what street and at what number it is to be carried on; when it shall commence, with sufficient information for the calculation of points under section 3-1-5 of this chapter; the full name of the applicant, and if a partnership, the full name of each member thereof, and if a corporation, the state of incorporation. If a license is sought by a person or partnership doing business under a fictitious name, such name shall also be set forth in the affidavit. (Prior Code § 13-2)
