(A) Every individual who is employed in connection with an adult use and who engages in services rendered by a body shop, model studio or massage establishment or who is a live entertainer or dancer shall be required to obtain an adult use employee license from the Village President or his or her designee.
(B) The information required on the original and renewal license applications shall include under oath:
(1) The applicant’s names including but not limited to actual name, stage names and aliases;
(2) The applicant’s age, date and place of birth;
(3) Present residence address and telephone number;
(4) Present business address and telephone number;
(5) State driver’s license or state identification number;
(6) Social Security number;
(7) Proof that the applicant is at least 18 years old;
(8) The applicant’s employment history for the preceding five years, including, but not limited to, any employment or ownership interest in an adult use; and
(9) Whether or not the applicant has been convicted of a misdemeanor or felony offense involving acts which are sexual crimes against children, child pornography, sexual abuse, rape or crimes connected with another adult use as defined in the Illinois Compiled Statutes.
(C) The license fee for an adult use employee license shall be $25 annually.
