(A) Applications for permits shall be completed on forms provided by the Street Commissioner, and shall be accompanied by the permit fee set forth herein, as follows:
(1) The permit fee shall be a minimum of $10.00 for any opening up to three feet by three feet, and $0.50 per lineal foot or fraction thereof for any portion of the excavation in excess of three feet in any dimension.
(2) In addition to the permit fee above, in cases where any underground service lines are to be uncovered, or are found to be uncovered, there shall be a charge of $0.50 per lineal foot or a charge of $5.00, whichever is greater.
(B) Permit applications shall contain the following information:
(1) Exact location of work to be done, with reference to street address, or intersection;
(2) Exact length, width, and depth of cut;
(3) Identity of job supervisor;
(4) Identity, address, and phone number of the permit applicant and any contractors and subcontractors who will be involved in completing the work covered by the permit application; and
(5) Anticipated start and finish dates.
