§ 111.02 APPLICATION FOR LICENSE.
   (A)   Applications for licenses by solicitors and peddlers shall be made to the Finance Department; then, forwarded to the Police Department for review. Applications shall be sworn and in writing and shall contain the following information:
      (1)   Name and physical description of applicant;
      (2)   Address;
      (3)   A brief description of the nature of the business to be conducted and the goods to be sold and, if the goods are farm products or provisions, a statement whether they are produced or grown by the applicant;
      (4)   If the applicant is employed, the name and address of the employer together with credentials establishing the exact relationship;
      (5)   The length of time for which the right to do business is desired;
      (6)   A statement as to whether or not the applicant has been convicted of any crime, including misdemeanors and violations of city or town ordinances; the nature of the offense; and the penalty assessed therefore; and
      (7)   Such other information as the city may deem necessary for the proper protection of the city.
   (B)   Applications shall be accompanied by not less than two photographs of the applicant, taken not more than six months prior to the date of the application. One photograph shall be attached to the application; in the event a license is issued, a photograph shall be attached to the license and made a part thereof. Photographs shall be of the head and shoulders of applicant and no less than two inches by two inches in dimension.
   (C)   The Police Department shall maintain a record of all applications for licenses remaining in force. In the event a license is not issued, the application shall be retained for a period of six months.
