§ 50.133 VIOLATION A CIVIL INFRACTION.

   If the Department discovers a cross-connection problem has occurred which results in contamination of the public water system, the City of El Mirage will forward a written cross-connection incident report within five business days to the local health authority containing the following information:

   (A)   Date and time of discovery of the unprotected cross-connection;

   (B)   Nature of the cross-connection problem;

   (C)   Affected area;

   (D)   Cause of the cross-connection problem;

   (E)   Public health impacts;

   (F)   Dates and texts of any public health advisories issued;

   (G)   Corrective action taken; and

   (H)   Date of completion of corrective action.
