APPENDIX B:  COMMENDATION AWARDS
   The following awards may be awarded to those deserving firefighters, police officers or citizens nominated and approved in accordance with the following procedures.
   (A)   Medal of Valor (blue/white/red/white/blue).  The Medal of Valor will be awarded to an Ellettsville Firefighter/Police Officer who conspicuously performs an act of gallantry and valor involving EXTREME risk of his or her own life while attempting a rescue. The deed performed shall have been one of personal bravery or self-sacrifice so conspicuously as to clearly distinguish the individual's act. Any firefighter/police officer may recommend award nominees to the Board of Chief Officers for review. The Board of Chief Officers will select nominees with final approval and presentation to be made by the Ellettsville Town Council.
   (B)   Medal of Bravery(blue/white/blue). The Medal of Bravery will be awarded to an Ellettsville Firefighter/Police Officer on the basis of an exemplary performance while COURAGEOUSLY risking his or her own life while confronting a dangerous situation. Any firefighter/police officer may recommend award nominees to the Board of Chief Officers for review. The Board of Chief Officers will select nominees with final approval and presentation to be made by the Ellettsville Town Council.
   (C)   Distinguished Service (blue/white/blue). The Distinguished Service Medal is awarded to any Ellettsville Firefighter/Police Officer who, while serving in any capacity with the Ellettsville Fire/Police Department, has distinguished himself or herself by exceptionally meritorious service to the government in a duty of great responsibility. The performance must be such as to merit recognition for service, which is clearly exceptional. Exceptional performance of normal duty will not alone justify an award of this decoration. Any firefighter/police officer may recommend award nominees to the Board of Chief Officers for review. The Board of Chief Officers will select nominees with final approval and presentation to be made by the Ellettsville Town Council.
   (D)   Medal of Merit (green/white/green/white/green/white/green). The Medal of Merit will be awarded to an Ellettsville Firefighter/Police Officer on the basis of outstanding performance and must have been such as to merit recognition for service that is FAR beyond the normal requirements of fire/police department duty. Any firefighter/police officer may recommend award nominees to the Board of Chief Officers for review. The Board of Chief Officers will have the final selection with the presentation to be made by the Ellettsville Fire Chief/Town Marshal.
   (E)   Certificate of Merit (yellow/white).  A Certificate of Merit will be awarded to an Ellettsville Firefighter/Police Officer on the basis of outstanding performance and must have been such as to merit recognition for service that is beyond the normal requirements of fire/police department duty. Any firefighter/police officer may recommend award nominees to the Board of Chief Officers for review. The Board of Chief Officers will have the final selection with the presentation to be made by the Ellettsville Fire Chief/Town Marshal.
   (F)   Firefighter/police Officer of the Year (red/white/blue/white/red). The Officer of the Year may be awarded to an Ellettsville Firefighter/Police Officer who has exhibited overall accomplishments and or participation during the year. Award nominees come from the board of officers.
   (G)   Professional Excellence Award (yellow/red). The Professional Excellence Award is to be awarded to an Ellettsville Firefighter/Police Officer on the basis of service rendered in an outstanding manner excelling in his or her regularly assigned firefighting/EMS/police duties. Any firefighter/police officer may recommend award nominees to the Board of Officers for review. The presentation is to be made by the Ellettsville Fire Chief/Town Marshal.
   (H)   Unit Commendation (blue). A Unit Commendation will be awarded to those Ellettsville Firefighters/Police Officer who, belonging to a single unit, act as one to accomplish a task in a meritorious manner. The presentation is to be made by the Ellettsville Fire Chief/Town Marshal.
   (I)   Good Conduct Award(red/white/red).  A Good Conduct Award shall be awarded in recognition of faithful, zealous and obedient service. The presentation is to be made by the Ellettsville Fire Chief/Town Marshal.
   (J)   Purple Heart (purple). The Purple Heart is awarded to members of the Ellettsville Fire/Police Department who are injured during an Emergency Response and requires the service of a physician and to those members who are killed in the line of duty or die of injuries received in the line of duty. The presentation is to be made by the Ellettsville Fire Chief/Town Marshal.
   (K)   Meritorious Service (white/blue/green/blue/white). The Meritorious Service Award is to be awarded to the Ellettsville Firefighter/Police Officer who has shown exemplary leadership, professionalism and strong commitment to quality service to the citizens who they protect. The presentation is to be made by the Ellettsville Fire Chief/Town Marshal.
   (L)   Public Service Award (yellow). The Public Service Award is to be awarded to an Ellettsville Firefighter/Police Officer who has demonstrated a direct contribution to the community in the form of donated time with a not-for-profit organization which is NOT in his or her job description. Any firefighter/police officer may recommend award nominees to the Board of Officers for review. The presentation is to be made by the Ellettsville Fire Chief/Town Marshal.
   (M)   Citizens Commendation (blue). The Citizens Commendation is to be presented whenever a civilian renders valuable or courageous assistance to members of the Fire/Police Department; particularly in cases of saving or attempting to save a life. A citizen selected for commendation shall be publicly honored by the department and presented with an appropriate citation during award ceremonies. Any firefighter/police officer may recommend award nominees to the Board of Officers for review. The presentation is to be made by the Ellettsville Fire Chief/Town Marshal.
   (N)   Public Safety Service Certificate of Merit (lt. blue/white). The Public Safety Service Certificate may be awarded to a member of the Police Department, Division of Public Safety, Municipal EMS or Fire Department on the basis of the individual's outstanding performance. This performance must have been such as to merit recognition for service which is beyond the requirements of his or her normally assigned duties. Award nominees may be recommended by any firefighter/police officer to the Board of Officers for review. The presentation is to be made by the Ellettsville Fire Chief/Town Marshal.
   (O)   Longevity (red). The Longevity Ribbon is to be awarded after five years of service and after each subsequent five year increments. The number of years served is to be inserted in center of bar - i.e.: 5,10,15 etc.
   (P)   Definitions.
      (1)   Fire Department is the Ellettsville Fire Department and Ellettsville Volunteer Fire Department.
      (2)   Fire Department Board of Chief Officers is the Chief, the Deputy Chief and the Assistant Chief.
      (3)   Fire Department Board of Officers is the Chief, Deputy Chief, the Assistant Chief, Captain and the Lieutenant.
      (4)   Police Department is the Ellettsville Police Department and Ellettsville Police Reserves.
      (5)   Police Department Board of Chief Officers is the Town Marshal and the Chief Deputy Town Marshal.
      (6)   Police Department Board of Officers is the Town Marshal, the Chief Deputy Town Marshal and the Sergeant.
