A charge of two dollars ($2.00) shall be made for each truckload of material or container load discharged into the municipal sewer system and shall be paid to the Director and credited to the sewer utility. Payments due the Town as a result of said charges shall be combined with a monthly report, properly sworn to before a Notary Public, to be filed with the Director at the end of each month's business and shall contain the following information:
(A) Date of report.
(B) Number of loads discharged into the sewer system.
(C) The following data on each load:
1. The name and address from whence the material was obtained.
2. Number of gallons involved.
3. Sewer discharge point.
4. Date and hour discharged into sewer.
(D) Signature.
(E) Affidavit and Notary Seal.
(F) Check in the amount of two dollars ($2.00) per load.
