§ 117.07  INFORMATION REQUIRED ON APPLICATION.
   The application for license shall be filed with the City Controller on a form or forms to be provided by the city and shall specify the following information:
   (A)   Name, address and telephone number of applicant;  (If applicant is corporation, firm, partnership or association, names, addresses and telephone numbers of the principal officers.)
   (B)   Date of application;
   (C)   Address of site where proposed activity is to be conducted;
   (D)   Type or character of activity;
   (E)   Name, address and telephone number of general manager of activity, if different from applicant;
   (F)   Trade name and general description of all devices to be used in conjunction with activity;
   (G)   Name and address of manufacturer, the names and addresses of the owner, lessor and lessee, and serial number (if any) of all amusement rides to be used in the activity;  (However, where it is impractical for the applicant to furnish the information requested in this division at the time of making of the application, the information may be furnished as soon as possible after issuance of the license and while the activity pursuant thereto is being carried on.)
   (H)   Recommendation of approval or denial by Chief of Police, Fire Chief and Health Officer;
   (I)   Date of recommendation of approval or denial by each of the officers;
   (J)   Date of issuance of license;
   (K)   Date of expiration of license;
   (L)   If recommendation of denial, specific factual reasons therefor;
   (M)   Recommendation of approval or denial, after applicant endeavors to correct noted defects and after reconsideration of officers recommending initial denial;
   (N)   Date of second recommendation; and
   (O)   Signature of applicant.
