§ 100.02 PERSONAL IDENTIFICATION CARDS.
   (A)   The Police Department is authorized to issue an identification card to any citizen of Elkhart County who has attained the age of 15 years of age who may request the card. The cards issued to any citizen who is at least 15 years of age, but less than 18 years of age, shall be considered juvenile identification cards and will be of a different color code than cards issued to any citizen over the age of 18.
   (B)   Application for these cards shall be on a form approved by the Chief of Police and shall contain any information which he or she specifies. With the application, the applicant shall submit proof of age in a form satisfactory to the Chief of Police, along with payment of a service charge in an amount to be determined by the Chief of Police, but in no case to exceed $5.
   (C)   Upon completion of an application, presentation of proof of age and payment of the service fee, the Police Department shall issue to the applicant an identification card in a form to be specified by the Chief of Police. The card shall include the name, address and a physical description of the holder, a head and shoulder photograph of the holder, the birth date of the holder and other information which the Chief of Police specifies.
   (D)   These identification cards shall be for the sole use of the person to whom it is issued, and use or attempted use of the card for any purpose by any other person shall constitute a violation of this section. If the person to whom the card is issued shall knowingly allow, promote, encourage or assist any other person to use or attempt to use the card for any purpose, his act shall constitute a violation of this section.
   (E)   Identification cards issued under this section shall remain the property of the city and must be returned to the Police Department within 14 days after written notice has been sent to the person to whom the card has been issued requesting the return. Failure to do so shall constitute a violation of this section.
   (F)   This section shall not be construed to require anyone to have, carry, apply for or show the identification card at anytime. The card shall be issued only at the request of and for the benefit of the holder.
   (G)   Any person violating the provisions of this section shall be punished by a fine of not more than $100.
